                                              Risk Prevention Guidelines for Student Extracurricular Activities

  I.	These guidelines were established in accordance with the “010 NTUNHS Rules on Safety Guidance for Off-Campus 
        Student Activities,as promulgated by the NTUNHS Extracurricular Activities Division. 

  II.	Student activities shall be declared to the Extracurricular Activities Division in advance; club activities shall 
        be handled in accordance with “090 Rules on the Guidance of Student Club Activities,” and class 
        activities shall be handled in accordance with "040 Rules on Student Classes Applying for Field Trips or Travel.”

  III.  Insurance: Insurance details must be certified by an insurance company  and relevant documents must be submitted along
         with a list of the insured parties to the Extracurricular Activities Division no later than 3 working days before
         the activity.
       1.	 One-day activities within the Greater Taipei Area (Taipei City and New Taipei City) do not require insurance; 
             however, activities with an element of danger, such as rafting, rock climbing, or mountaineering, must be insured. 
             Activities in other areas must be insured. 
       2.	 Activities over multiple days must be insured.

  IV.   Protective equipment and first aid kits: Based on their design and attributes, student activities must be supplied with
         sufficient safety and protective equipment and first aid kits; examples include first aid kits for general field trips,
         fire extinguishers for campfire parties, crash mats for rock climbing, and flotation devices for water activities. 

  V.    Activities with higher levels of danger must be accompanied by at least one certified counselor, coach, instructor, 
         or lifeguard depending on the nature of the activity; examples include a mountaineering guide, nurse, rock climbing 
         instructor, or swimming lifeguard. 

  VI.  Overnight or parental consent forms: 
        1.	  For class or club activities that span multiple days and involve spending the night away from home, all attendees
              must submit a parental consent form for class trips or an overnight consent form for club activities. 
        2.	  Given the safety issues associated with overnight activities, students aged 20 years and older must submit 
              a consent form as prescribed above and provide their parent’s or guardian’s information. 
              If the student, age 20 or older, fill in their own name as their guardian, they must provide another emergency
              contact person who the university can contact in the event of an emergency. Parents or guardians must 
              be a relative or a legal spouse. 

  VII.   If adverse weather conditions or natural disasters are announced prior to the activity, such as in the event of a 
           typhoon warning or heavy rain advisory, activities in hazardous areas such as the coast, the mountains, rivers,
          or disaster-affected areas must be terminated immediately. In the event of a natural disaster and the announcement
         to suspend work and classes during the activity, the activity shall be terminated immediately. 

  VIII.	Transportation 
     A.	If the activity involves travel using public transportation, safety documents are not required; however, the 
           embarkment and destination stops must be written in the activity plan and application.
     B.	If the activity involves travel using privately chartered transportation, such as a chartered bus, the following 
           information must be submitted to the Extracurricular Activities Division no later than 7 days before the activity
          (after the information has been submitted,the driver and vehicle can no longer be changed on the day of the activity): 
          1.	A copy of the driver’s license
          2.	A copy of the vehicle license and the driver’s license, which must be issued within the past 5 years 
          3.	Proof of third-party insurance
          4.	The third-party insurance coverage must end on the day of the activity or on a later day . 

  IX.	  Food safety
    A.	 If alcohol is provided at the activity, the attendees must each submit an “Alcohol Safety and Risk 
            Awareness Statement” confirming their understanding of alcohol-related precautions and safety issues.
    B.	 Alcohol: If alcohol is provided at an activity, the advising teacher or a counselor from the Extracurricular 
            Activities Division must be consulted on safety measures; the activity plan must also include details on what 
            alcoholic beverages will be served, how they will be distributed, and what safety measures and solutions are in place.
           The activity organizers must ensure that every attendee arrives home safely. 
    C.	Food safety: Adequate attention must be paid to food cleanliness, food safety, and hydration during the activity. 

  X.   Gender equity: The activity must not be designed to violate good social customs and must not involve sexual harassment, 
         sexual bullying, or images or activities involving a sexual nature or gender discrimination; any behavior that is 
         out of order is strictly prohibited.

  XI.   Renting venues for off-campus activities:
    A.	The rented venue must have passed their safety inspections; information about the venue must be submitted to the
            Extracurricular Activities Division no later than 7 working days before the activity. 
    B.	If the venue is 100 pings or larger, the following documents must be provided:
          1.	A copy of the business registration
          2.	A copy of the most recent fire safety certification
          3.	A copy of the public liability insurance for the property, unless the lessor is a government department.
    C.	If the venue is smaller than 100 pings, the following documents must be provided:
          1.	A copy of the business registration
          2.	A copy of the public liability insurance for the property
          3.	A map and photos depicting the fire extinguishers, emergency lighting, and fire alarms; the photos of the fire
                extinguishers must show the expiration dates.
          4.	A schematic of the venue, with the fire extinguishers, emergency lighting, fire alarms, and emergency exits
                clearly marked.
    D.    A rental agreement must be signed when renting a venue, stipulating measures such as the unconditional delay or
            cancellation of the venue rental, with no extra fees, in the event of adverse weather or disasters or in the 
           event that the government orders the suspension of work and classes.

  XII.	Open-flame equipment: Open-flame performances refer to performance activities that produce any flames or sparks. 
        A.	 Indoor venues: The use of flammable materials with any fire or sparks is strictly prohibited.
        B.	 Outdoor venues: Open-flame performances in outdoor venues must be organized in compliance with the Rules on the 
                 Safety and Management of Open-Flame Performances as promulgated by the Ministry of the Interior.
       C.  	 If the activity involves any open flames, please endeavor to replace hazardous materials with safe equipment or 
                materials, such as replacing fuel-burning fireballs with LED lights.
       D.	    If open-flame equipment is necessary, safety measures must be discussed with the advising instructor or a 
                counselor from the Extracurricular Activities Division and stipulated in the activity plan. 

  XIII.	 The venue rental business and activity organizers are required to purchase public liability insurance at an 
            amount commensurate to the nature and scale of the activity.

  XIV.	These guidelines shall be carefully read by all faculty and students involved with organizing the student
            activity and shall serve as a basis for activity safety. Any matters that were not addressed sufficiently or 
            at all may be stipulated in the activity plan during the application.

  XV.	These guidelines and any amendments shall be implemented following approval at the Student Affairs Meeting.

一、依本校課外活動指導組「010國立臺北護理健康大學校外學生活動安全輔導辦法」
    訂定本要點。

二、學生辦理活動,社團活動依【090學生社團活動輔導辦法】、班級活動依【040學生
    班級郊遊、旅遊申請辦法】活動前向學生事務課外活動指導組申請備查。

三、保險: 保險資料須有保險公司核章之證明文件及被保險人名冊,最晚於活動前3個
         工作日送到課外活動指導組。
   (一)	一日活動:地點在大臺北地區(臺北市及新北市)不用保險,其他地區則須要保險。
                但活動設計具如有危險性質如泛舟、攀岩、登山.等等大臺北地區仍須保險。 
                
   (二)	二日以上活動必須保險。

四、防護裝備急救器材: 針對活動設計及特性不同,必須準備足夠的安全防護裝備及
   急救器材,如一般郊遊備醫藥箱、營火晚會備滅火器、攀岩備防護墊、水邊戲水
   備救生圈…等器材。

五、依其活動性質不同,危險性較高的活動須至少有一位具合格輔導員、教練(師)或
   救生員隌同,如登山響導、護理師、攀岩教練、游泳救生員或教練…等。

六、過夜同意書或家長同意書: 
   (一)	班級或社團活動辦理二天以上,到外面過夜時每位參與人員必須繳交
        班級旅遊【家長同意書】;社團活動【過夜同意書】。
   (二)	外出過夜牽涉安全處理問題,(含)二十歲以上者須繳交上述同意書及
        須填寫家長或監護人資訊;若(含)二十歲以上者填寫自己為監護人時,
        須提供另一位緊急聯絡人資訊俾利校方緊急狀況時可聯繫。
        家長或監護人身份必須為直系、旁系親屬或具法律效力之配偶等人。

七、活動前若預知天候因素、天災等(海上、陸上颱風警報及豪大雨特報、地震
   等…),為維護安全,於海邊、山區、河川地或災區等危險警戒區域,
   應立即終止進行活動。活動時間如遇天然災害公告停止上班及上課作業,
   應立即終止進行活動。

八、交通工具
  (一)	搭乘大眾運輸工具: 免附安全資料,但須在企劃書及活動申請書說明
        搭乘工具起迄地點。
  (二)	搭乘私人承攬運輸工具(如遊覽車..),以下資料必須最晚於活動前7個
        工作日送到課外活動指導組。
        已送之駕駛資料及車輛資料,不可在活動當天臨時更換駕駛及車輛。
     1.	駕駛人駕照影本
     2.	5 年內新車行照影本、司機駕照影本 
     3.	第三責任險保險資料證明
     4.	第三責任險,保險截止日必須是在活動當天或之後的日期 

九、飲食安全
  (一)	活動有提供酒精性飲料時,參加成員每人必須於活動前繳交
        【活動提供酒精安全暨風險事項了解確認書】,正確了解酒精性飲料
        應注意事項及安全。
  (二)	酒精性飲料:活動辦理提供酒精性飲料時,必須與指導老師或課外活動指
        導組輔導老師共商安全措施,並於企劃書說明酒精性飲料、發放方式、
        安全措施及解決方法,主辦單位須確保每位參與同學平安到家。
  (三)	食物衛生:活動期間必須注意食物清潔、食品安全及水份補充。

十、性別平等:活動設計不能有違背善良風俗,避免有性騷擾、性霸凌,或具有性意味
   或性別歧視之圖文或活動,並嚴防脫序之行為。

十一、校外活動場地租借事宜:
  (一)	場地租借必須以安全合格為主,場地相關資料最晚於活動前7個工作日
        送到課外活動指導組。 
  (二)	100坪以上須附:1.廠商營利事業登記證明影本。2.近期消防安全
        檢測通過證明影本。3.產物公共意外責任保險單影本;政府部門則無須檢附。
  (三)	100坪以下須附:1.廠商營利事業登記證明影本。2.產物公共意外責任
        保險單影本。3.滅火器、緊急照明燈、火災警報器三樣的位置圖及照片,
        滅火器照片上要標明有效日期。4.場地位置圖及標明滅火器、緊急照明燈、
        火災警報器、逃生位置。
  (四)	租借場地時,必須訂定合約書,並註明如遇天候因素、天災等或公告停止上班
        及上課等非人為因素,無條件將租借時間延後或如時間無法配合將可取消場地,
        不另支付賠償費用等等措施。

十二、明火設備:所稱明火表演,指以產生火焰、火花或火星等方式之表演活動。
  (一)	室內場地:禁止使用任何有火或火花之易燃物。
  (二)	室外場地:務必遵守內政部頒布【明火表演安全管理辦法】。
  (三)	活動設計如有明火請盡量以安全之設備材料取代具危險性之材料,如火舞表演可以LED燈
        取代燃油火球。
  (四)	如須明火設備,必須與指導老師或課外活動指導組輔導老師共商安全措施後,於企劃書
        中明訂之。
 
十三、應依活動性質及規模要求出租場地單位、承辦活動或研習單位投保公共意外責任險及
     適足保險金額。

十四、本要點於學生活動辦理相關師生於活動規畫辦理詳閱,作為活動安全基本依據,
     如有未列出及不足部份,可於活動申請時於活動企劃書中明訂。

十五、本要點於學生事務會議通過後實施,修改亦同.
